The length of time from exposure to a patient's first developing symptoms of PPH or PAH varies greatly. Thus, it is often several years after the first symptoms appear (usually shortness of breath) before PPH is diagnosed.

Normal small pulmonary artery with typically thin muscular wall.
Some of the changes that can occur in primary pulmonary hypertension or pulmonary arterial hypertension:
Wall of small pulmonary artery thickens.
Fibrous, or scarred, tissue appears on inner wall of small pulmonary artery.
Bands of scarred tissue build up on inner wall of small pulmonary artery, substantially narrowing the blood vessel.
